Kindergarten registration set Kindergarten registration for the 2001-2002 school year will be at the elementary school in your district April 19 from 9 - 11 a.m. and 4 - 6 p.m. A child must be 5 years old on or before Sept. 1 to register. Parents will need to provide the school with the following: 1. Certified birth certificate (must have imprinted raised seal). 2. Child's Social Security card. 3. One of the following documents: property deed (Warranty/Security), original signed lease agreement, or affidavit of residency (may be obtained 8 - 11 a.m. and 1 - 4 p.m. by contacting Beverly Pittman, LaFayette Educational Center, 205 LaFayette Ave., Fayetteville, 770-460-3990, ext. 122). 4. Current gas or electric bill displaying your name and address of service. 5. Photo ID of parent/guardian registering student. By the beginning of school the following information is required: 1. Immunization record on the Georgia state form 3231. 2. Eye, ear and dental records on the Georgia form 3300.
