Hospital offering free workshop Fayette Community Hospital, in partnership with Georgia Health Decisions, is hosting a comprehensive workshop for you and your family to learn about advanced directives and other legal documents concerning end-of-life healthcare decisions. The seminar is free and open to the public. Participants will be provided with the tools essential for planning ahead, The Critical Conditions Planning Guide and the Directive for Final Health Care form. During the workshop, attendees will learn how to complete a Directive for Final Health Care form, which combines a living will and a durable power of attorney for healthcare. The best way to ensure that your wishes are fulfilled is to let your family know now.
